Pay: $22.42-$23.79/hour, depending on experience
Probationary Period: 90-Day Review
Reports To: Clinical Program Lead; Director
Position Summary
The Community support Specialist supports Pathways Services LLC in providing community-based case management, client support, and lifeskills programming for referred individuals in Delta and Montrose Counties. This position works directly with clients involved in municipal court, probation, community-based referral systems, and other approved pathways to services.
The position includes direct client support, coordination with referral partners, documentation, resource navigation, and assistance with the development and facilitation of evidence-based and psychoeducational groups. This role is ideal for an individual with experience working with justice-involved individuals, high-needs populations, and community systems, and who is comfortable building professional relationships with courts, probation, and partner agencies.
Essential Duties and Responsibilities
Case Management and Client Support

Provide direct support and case management to assigned clients

Conduct client check-ins and ongoing follow-up

Assist clients in identifying barriers, goals, and action steps

Support referrals to community-based services and resources

Maintain appropriate documentation, case notes, and service records

Communicate professionally with referral partners as appropriate and authorized

Help clients navigate appointments, service expectations, and program participation
Program Coordination

Assist with coordination of referrals from municipal court, Riverbend, probation, and other approved sources

Support communication and relationship-building with community partners

Help track participation, engagement, attendance, and progress

Assist in identifying service gaps and opportunities for program growth

Support implementation of Pathways programming in alignment with organizational goals and approved curricula
Group Facilitation

Assist in planning, organizing, and launching psychoeducational groups

Facilitate or co-facilitate approved group programming

Complete attendance tracking, session notes, and related documentation

Conduct participant reminders and follow-up related to group attendance

Help maintain fidelity to approved curricula and program expectations

Participate in training and certification processes for assigned curricula as needed
Other Duties and Expectations

Attend meetings related to client care, referral coordination, or program development as needed

Maintain professional boundaries, confidentiality, and ethical practice at all times

Perform other related duties consistent with the scope of the position as assigned
Preferred Qualifications

Experience working with justice-involved individuals, high-needs populations, or community-based human services

Strong professional judgment, interpersonal skills, and boundaries

Familiarity with case management, referrals, documentation, and service coordination

Ability to build and maintain productive working relationships with courts, probation, service providers, and community partners

Experience facilitating groups or willingness to be trained in approved curricula

Strong organizational skills and ability to work independently

Driver's License, Reliable transportation and ability to travel locally as needed
